This form walks you through reporting electronic resources access and technology issues.

  1. If this is a general technology issue (with a service desk or public computer, a printer/copier/scanner, a group study room monitor, or the WiFi) answer No to Are you having an issue related to electronic resources? and Describe the problem in as much detail as possible.
  2. If this is an e-resource access issue, answer Yes to Are you having an issue related to electronic resources? AND ANSWER ALL THE REQUIRED QUESTIONS THAT ARE SHOWN.
  3. If a patron needs help with the issue, add their name and contact email.
  4. Upload any screenshots that show the problem.
